Designation of Beneficiary Form
The Designation of Beneficiary Form protects your dependents from financial difficulty in the untimely event of your death. The beneficiary will receive funds due to you from the university. If you have not completed a Designation of Beneficiary form, you may do so at anytime. You may also update previous designations at anytime.
