Performance Evaluation
Overview
Performance evaluation is a necessary and benefical process, which provides annual feedback to staff members about job effectiveness and career guidance. The performance review is intended to be a fair and balanced assessment of an employee's performance. To assist supervisors and department heads in conducting performance reviews, the HR-Knoxville Office has introduced new Performance Review forms and procedures for use in Knoxville.
A Performance Review Instruction Form (PDF) will help you complete and submit the forms. The most significant change is the addition of the Performance Review Summary Form (PDF).
The Performance Review Summary Form is designed to record the results of the employee's annual evaluation. During the performance review meeting with the employee, use the Performance Review Summary Form to record an overall evaluation in:
- accomplishments
- service and relationships
- dependability
- adaptability and flexibility
- and decision making or problem solving.
Submit the completed Performance Review Summary Form (and any requested back-up documentation needed to support the final evaluation) to:

Back-up documentation may be the Performance Review Detail Form, your college's or department's detailed evaluation instrument, and/or a memo detailing and justifying the review rating.
Description of Forms
- Performance Review Detail Form (PDF) - Complete this form as back-up documentation to assist you in completing the Performance Review Summary Form. The categories are grouped to match the categories on the Summary Form. For each item, choose the response that most closely matches your views of the employee's performance. Your college or department may use different forms as back-up documentation for evaluating your staff. (PDF version)
- Goals and Objectives Form (PDF)- List goals and objectives for the employee to meet during the coming year. Include time frames for when goals are to be met. The evaluation section is to be used to evaluate the goals from previous reviews. (PDF version)
- Administrative Peer/Staff Review Form (PDF)- Use this form to obtain feedback from employees on the supervisor's or respective administrator's performance. (PDF version)
